I take full responsibility for my personal health and progress on my nutrition plan. I acknowledge that positive change in health can take time. I understand that a Registered Holistic Nutritionist is not a medical doctor, naturopath or dietician, and thus is not designated to diagnose, cure or prevent disease, pain, deformity, injury or mental and physical conditions of any kind. I understand and acknowledge that the services provided are at all times restricted to consultation on the subject of health matters intended for general well-being.
CONSENT TO TREATMENT:
I consent to receive treatment as prescribed by the practitioners I am seeing for all my scheduled appointments. I understand that I can withdraw my consent at any time should I not understand the treatment, or not agree with the treatment being given. As such, I will let the practitioner know if I withdraw my consent. Until such time that consent is withdrawn verbally, consent is implied.
PRIVACY AND SHARING OF INFORMATION:
All information shared within the professional relationship will be held with the strictest of confidence. Information may be shared at the client's request with a medical doctor, naturopathic doctor or any other healthcare practitioner the client deems appropriate. Confidential information will only be shared if the law requires it.
Your appointment time has been reserved for you. A late cancellation or missed visit leaves a hole in the therapists' day that could have been filled by another patient. As such, we ask that you provide us with 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, will be charged the price of a full appointment. There will be a 50% cost for a make-up appointment.
Once the first appointment is completed, the cost of the appointment is non-refundable.
